Guest Liability Release Forms
All guests must sign release forms prior to entering the Space, and be accompanied by a current Akron Makerspace member OR be a registered attendee of a class or event. Guests are NOT allowed to use tools or materials at Akron Makerspace outside of what is provided/used for the class or event.
If you are attending a class, the “AMS Member” section should be completed by your instructor.

- If the Minor will be accompanied by an AMS member that is not their parent or guardian, the guest release form must be completed with their parent’s/guardian’s signature prior to entering the Space.
